If confirmed that you are HIV positive (from a doctor, health department or from us) a referral is made to our case manager.
The case manager conducts a thorough assessment to see what services you may need and will work out a care plan to aid in your compliance. We provide referrals to other community services to assure you get all the help you need. With your consent, we also can meet with family members to help guide them through the ways they can help you.
